We accept PPO and some HMO insurance for some of our providers and services. Please reference the chart below. For qualified services we bill your insurance company for you. You are responsible for your co-pay and deductible. Your benefits depend on your individual plan. Submit your plan details and and we will help you find out about your benefits. For the uninsured or under-insured a reduced rate may apply. All of our providers can provide a superbill if you prefer to pay fees up front and make your own claim with your insurance carrier.
